Currently serving as a Proactive Care Practitioner at Seacroft Proactive Care Team, Seacroft PCN, the team are responsible for Seacroft Blood Pressure Awareness project, Frailty Reviews and community engagement, driving innovation and achieving exceptional results.

Katie has a solid background in frailty in primary care, making them a valuable asset to their team. The team’s expertise has led to them presenting their work at the British Geriatric Society annual conference 2024.
